    People, please be warned and go to a physiotherapist for a custom therapy regimen based on your condition. I did this regimen exactly and the pain has increased exponentially. Especially the lunges are a no go. Stretching in the early phases of a quad tendinitis will damage your tendon more depending on how inflamed the tendon is. Everyone is different, there is no one-fit-for-all regimen

    • @MichaelBraccio
      @MichaelBraccio 6 дней назад

      Sorry to hear that you had a flare up. With rehab there are so many different variables to consider like frequency, duration, and intensity of the exercises along with when to progress to another exercise. So of course having someone qualified to guide treatment is essential. For clarification, stretching the tendon does not produce enough load to damage the tendon. It can lead to irritation, but not damage.

  • @mrpentium
    @mrpentium 7 дней назад

    Can you please explain why we want to put loading on a tendon that is already inflamed and in a stressed condition ?

    • @MichaelBraccio
      @MichaelBraccio 6 дней назад

      Of course! It depends if it was caused by a sudden increase in loading or more of a chronic overload. Sudden overload we want probably reduce the load to allow the tendon to calm down. But for chronic overload, there are altered proteins in the tendon which are drawing more fluid into the tendon leading to the swelling/inflammation. So gradually loading helps to change this protein composition. Also exercise can have a pain relieving effect when dosed appropriately so short term this can be helpful.

    • @mrpentium
      @mrpentium 6 дней назад

      very interesting. Is there something I can Google to learn more about the protein composition that changes with exercise ? Is this another way of saying inflammation alleviation?

    • @MichaelBraccio
      @MichaelBraccio 5 дней назад

      Kind of. The inflammatory process involves different pathways, so they are often absent in tendinopathy. But in both cases, fluid can accumulate in the area. This article might be helpful: www.ncbi.nlm.nih.gov/pmc/articles/PMC6737558/

    Worst stretch ever. Stretching muscles while under load. As a gymnast in the 1980's we learned to sit on the floor with your toes against the wall, or point toes to ease it or pull them toward you if you want to make it more difficult. Better still is to lay on the floor with legs up a wall, bottom touching the wall. Then pull toes back toward the ground. You can also split the legs to do groin stretch in a controlled way.

    • @MichaelBraccio
      @MichaelBraccio 11 дней назад

      Hi David, This is a motor control exercise that helps with shifting the weight back, which can improve the toe touch. So, I don't think it qualifies as the "worst stretch ever." Additionally, pointing the toes up during a typical hamstring stretch increases the stress on the sciatic nerve rather than increasing the stress on the hamstrings.

    It's not only pain its to repair the tendons that are torn off to make them more stable.

    • @MichaelBraccio
      @MichaelBraccio 10 дней назад

      Sort of. Surgical repair can help with pain/function, but the retear rate is around 41%. So improvements aren't necessarily attributed to an intact rotator cuff. Here's the research article: pubmed.ncbi.nlm.nih.gov/35179393/

    I thought leg extension are hard on knee when you have patellarfemoral pain. Also if I have leg extension at gym is it better to use that start at 5lbs each single leg and progress? Shouldn't leg extensions be done to only 45 degrees.

    • @MichaelBraccio
      @MichaelBraccio 10 дней назад

      The compressive loads can be irritating, but it depends on the tolerance (weight plus range of motion). So it's about finding the combination of load and range of motion that is tolerated and gradually increasing.

  • @joshuahankins2613
    @joshuahankins2613 18 дней назад

    Also I found an appendix for the study they would add either bands or ankle weight increments for progression within the exercises.

    This is a helpful progression. However, having worked through similar progressions over the past two years, I find that the tricky part is being able to judge when to move onto the next phase. More often than not, my progress isn't steady, but more like a series of ups and downs, depending on the day, the activity, and whatever else I'm doing outside of the gym.

    • @MichaelBraccio
      @MichaelBraccio 10 дней назад

      Unfortunately progress isn't linear with rehab because there are a lot of variables that can influence pain day to day. For example, sleep and stress can make some exercises that were tolerated previously become more painful.
